What this notice is about

You still have unpaid federal employment tax. We issued a levy to collect your unpaid taxes.

What you need to do

Pay your balance in full to avoid paying more in penalties and interest.

  • If you can’t pay the full amount, learn more about how to set up a payment plan. You must be current on your filings to apply for an installment agreement.
  • If you’ve already paid the balance or have further questions about your account, call the number on your notice.

You may want to

  • Request an appeal to the proposed levy and balance due. For more information, see Form 12153, Request for a Collection Due Process or Equivalent Hearing PDF and Collection Due Process (CDP) FAQs.
  • You can also review general information on IRS levy procedures.

What to do next

  • Write down the date on the letter today.
  • Look the number up on irs.gov yourself. Don’t trust a link or phone number from anyone else.
  • If it says you owe money or proposes changes to your return, take it to a licensed CPA, EA or tax attorney before that date.

Not advice. Not a filing. Verify with a licensed professional. Rules change by year and facts.
